S - F) Create "Letter of Undertaking For UTAR by Parents or Guardian"

Steps

Step 1) Select your "Industrial Training Pre-Registration" and complete the rest of the information."

After entering the name of "Second Parent / Guardian" you will then be able to enter the IC number and guardian relationship.

Enter the contact address and contact phone number.

Step 2) Check the check box "I acknowledge the information providd is true and correct" and view the sample letter

Step 3)  Save and click the "Printer icon" to view the generated letter.
